AI scribe for couples therapists

Facts checked on 17 October 2026.

MentalNote is an AI scribe for couples therapists: record a couples or family session in your browser, get a speaker-labeled transcript and one draft note in DAP, BIRP, GIRP, SOAP or Progress format, or in your own custom format, on a free plan or Premium at $99 a month, with a BAA on every plan. You decide whose record the note belongs in.

Couples sessions are hard to write up: two people, two versions of the same week, and a pattern you are tracking between them. MentalNote listens to the session (with consent from everyone in the room) and drafts one note in your structure, with a transcript labeled by speaker so you can check who said what. You review it, edit it and paste it into your EHR.

5 free scribe sessions a month. No credit card. BAA on every plan.

The short answer

  • Record the session in your browser, in the room or on a telehealth call in a browser tab, and get a draft note when the session ends.
  • Pick the format you chart in: DAP, BIRP, GIRP, SOAP, PIRP, Progress, Intake / Biopsychosocial or Treatment Plan, or your own structure with custom instructions.
  • It is told not to fill gaps. The app instructs the AI to write "Not documented in session" for anything the session did not cover, and not to add a denial, a diagnosis or a risk rating you did not state. Check every draft.
  • Start free: 5 scribe sessions a month, no time limit, no credit card. Premium is $99 a month or $990 a year.
  • MentalNote is HIPAA-compliant, with a BAA included on every plan, the free plan too. Many AI scribes offer only a time-limited trial.

Who it is for

Solo clinicians and small practices that want to sign up today, pay a published price and start without a sales call or an IT project. Premium is for solo clinicians. Practices can apply for the Practice Pilot.

Built for couples and family sessions

  • Speaker labels: the transcript is labeled by speaker, so you can check each partner's words before you sign. Labels are generic, so confirm which speaker is which.
  • One note per recording: each recorded session produces one draft. If your practice keeps a note in each partner's chart, or names one identified client, edit the draft for that record before you file it.
  • Your model, your headings: MentalNote has no built-in Gottman or EFT template. Save one custom format (up to 500 characters) with the headings you use, such as cycle, interventions and each partner's response. It controls structure only.
  • Interventions you name go in the Intervention section. The app tells the AI not to add interventions, scores or homework that were not in the session.
  • Consent first: the web app asks you to confirm consent before every recording.

Note formats you can generate

What a couples session BIRP draft looks like

A human-written, shortened illustration based on a fictional session (no real client). It is not verbatim app output, and your drafts will vary with what is said.

Fictional example: Couples session 6, pursue-withdraw cycle (BIRP)

Behavior

Partner A reports two arguments this week about weekend plans, both ending with Partner B leaving the room. Partner B states: "When the questions start, I just shut down." Both partners report one calm conversation on Sunday.

Intervention

Clinician mapped the pursue-withdraw cycle from the Saturday argument and coached each partner to name the feeling under the reaction. Practiced a 20-minute pause with an agreed return time.

Response

Partner A named feeling shut out rather than angry. Partner B agreed to say "I need 20 minutes" before leaving the room. Both partners described the in-session practice as useful.

Plan

Continue weekly couples sessions. Homework: use the pause signal and one 15-minute check-in. Review the cycle map next session.

Risk content is not in this draft because it was not discussed in this fictional session. MentalNote is instructed never to write "denies SI" for a question you did not ask.

Whose note is it?

Couples and family therapists chart in different ways. Some keep one note for the couple, some write the note in the identified client's record, and some keep a separate note for each partner. MentalNote does not decide this for you: it drafts one note per recording, describing what each person reported and how they responded.

  • Identified client: if one partner is the client of record, edit the draft so it focuses on that client and refers to the other partner by role.
  • Separate charts: copy the draft into each record and remove what does not belong there before you sign.
  • Billing: check your payer's rules for couples and family sessions with and without the client present. The draft does not choose a code for you.

Writing a custom format for your model

Your custom format controls the structure and headings of the note. The app's documentation rules still come first: only what was said, "Not documented in session" for gaps, no invented scores and no diagnosis you did not state. Some starting points:

  • Gottman-informed: presenting conflict, skills practiced (for example softened start-up or repair attempts), each partner's response, homework.
  • EFT-informed: cycle tracked, emotions accessed, enactments, each partner's response, plan.
  • Family session: who attended, family pattern observed, interventions, each member's response, plan.

What it will not do

These are instructions the app gives the AI for every therapy note. They are meant to reduce errors, but they are not a guarantee. Review every line.

What can go wrongWhat MentalNote instructs the AI to do
A risk statement that was never asked ("denies SI")Include risk and safety content only if it was explicitly discussed, and never write that the client denied something the session did not ask about.
A diagnosis the AI guessedGive no diagnosis, diagnostic impression or code unless you stated it in the session.
An intervention you did not useUse only what the session contains: no invented interventions, scores, dates or homework.
A gap filled with typical contentWrite "Not documented in session" for anything the session did not cover.

How it works

  1. Confirm you have your client's consent. The web app asks you to confirm it before every recording.
  2. Record in the browser, in the room or on a telehealth call in another tab.
  3. When the session ends, choose a format and get the draft, with the speaker-labeled transcript beside it.
  4. Review and edit every line, then copy or export it into your EHR.

Prefer Microsoft Word? The optional Word add-in drafts a therapy note from a short typed summary, with no recording.

Frequently asked questions

Can it tell the partners apart?

The transcript is labeled by speaker, so you can see who said what. Labels are generic, so check which speaker is which and every attribution before you sign.

Does it write a separate note for each partner?

No. Each recording produces one draft note. If you keep a note in each partner's record, or chart under one identified client, edit the draft for that record.

Does MentalNote have a Gottman or EFT template?

Not a built-in one. Save your own custom format, up to 500 characters, with the headings you use. It sets the structure; the no-invention instructions still apply.

Is there a free AI scribe for couples therapists?

Yes. MentalNote's free plan includes 5 scribe sessions a month, with no time limit and no credit card. The BAA covers the free plan too. Premium is $99 a month or $990 a year.

Do both partners need to consent to recording?

Get consent from everyone in the session, and follow your state's recording law and your licensing board's rules. The web app asks you to confirm consent before every recording.

Is MentalNote HIPAA compliant?

MentalNote is HIPAA-compliant, with a BAA included on every plan. You accept it in the app before your first session.
