AI treatment plan generator

Facts checked on 16 October 2026.

MentalNote is an AI treatment plan generator for therapists and counselors: record the intake or treatment planning session in your browser, choose Treatment Plan, and get a draft with presenting problems, diagnosis (only if you stated one), long-term goals, measurable short-term objectives, interventions, strengths and barriers, client participation and a review date, on a free plan or Premium at $99 a month, with a BAA on every plan.

Treatment plans are the note everyone puts off: they need measurable objectives, they have to match the intake, and payers want them updated. MentalNote drafts the plan from the conversation you actually had with your client, so the goals are in the client's words and the objectives are tied to what you agreed. You review it, edit it and paste it into your EHR.

5 free scribe sessions a month. No credit card. BAA on every plan.

The short answer

  • Record the session in your browser, in the room or on a telehealth call in a browser tab, and get a draft note when the session ends.
  • Pick the format you chart in: DAP, BIRP, GIRP, SOAP, PIRP, Progress, Intake / Biopsychosocial or Treatment Plan, or your own structure with custom instructions.
  • It is told not to fill gaps. The app instructs the AI to write "Not documented in session" for anything the session did not cover, and not to add a denial, a diagnosis or a risk rating you did not state. Check every draft.
  • Start free: 5 scribe sessions a month, no time limit, no credit card. Premium is $99 a month or $990 a year.
  • MentalNote is HIPAA-compliant, with a BAA included on every plan, the free plan too. Many AI scribes offer only a time-limited trial.

Who it is for

Solo clinicians and small practices that want to sign up today, pay a published price and start without a sales call or an IT project. Premium is for solo clinicians. Practices can apply for the Practice Pilot.

What goes in the plan

  • Eight sections: Presenting Problems, Diagnosis, Long-Term Goals, Short-Term Objectives, Interventions, Strengths and Barriers, Client Participation, Review Date.
  • Traceable goals: the app tells the AI that goals, objectives and interventions must come from what was discussed. Objectives are written to be measurable without inventing baseline data.
  • No guessed diagnosis: the Diagnosis section is filled only if you stated one in the session.
  • Client participation records how the client took part in setting the goals, which many payers and auditors look for.
  • Then keep each session on track with GIRP notes, which tie every session to the plan's goals.

What a treatment plan draft looks like

A human-written, shortened illustration based on a fictional session (no real client). It is not verbatim app output, and your drafts will vary with what is said.

Fictional example: Treatment plan after intake for social anxiety

Presenting Problems

Anxiety in social and work situations, with avoidance of team meetings and phone calls.

Low mood on days after avoided events.

Diagnosis

Social anxiety disorder, as stated by the clinician.

Long-Term Goals

Client will take part in work meetings and social events with manageable anxiety. In the client's words: "I want to speak up at work without dreading it all week."

Short-Term Objectives

Within 8 weeks, client will speak at least once in 3 of 4 weekly team meetings, tracked on a weekly log.

Within 6 weeks, client will make 2 work phone calls a week instead of emailing, rating anxiety before and after.

Interventions

Weekly CBT: psychoeducation on the anxiety cycle, cognitive restructuring and a graded exposure hierarchy built with the client.

Strengths and Barriers

Strengths: motivated, supportive partner, stable job. Barriers: long work hours, and a habit of cancelling plans at short notice.

Client Participation

Client identified the goals and helped rank the exposure hierarchy, and agreed to the plan.

Review Date

Review in 90 days.

The diagnosis appears only because the clinician stated it. Baselines and targets come from what the client reported, not from the AI. Review and adjust every objective before you sign.

What it will not do

These are instructions the app gives the AI for every therapy note. They are meant to reduce errors, but they are not a guarantee. Review every line.

What can go wrongWhat MentalNote instructs the AI to do
A risk statement that was never asked ("denies SI")Include risk and safety content only if it was explicitly discussed, and never write that the client denied something the session did not ask about.
A diagnosis the AI guessedGive no diagnosis, diagnostic impression or code unless you stated it in the session.
An intervention you did not useUse only what the session contains: no invented interventions, scores, dates or homework.
A gap filled with typical contentWrite "Not documented in session" for anything the session did not cover.

How it works

  1. Confirm you have your client's consent. The web app asks you to confirm it before every recording.
  2. Record in the browser, in the room or on a telehealth call in another tab.
  3. When the session ends, choose a format and get the draft, with the speaker-labeled transcript beside it.
  4. Review and edit every line, then copy or export it into your EHR.

Prefer Microsoft Word? The optional Word add-in drafts a therapy note from a short typed summary, with no recording.

It can write a draft. MentalNote drafts a treatment plan from your recorded session, with goals, objectives and interventions taken from what was discussed. You review, edit and sign it.

Are the objectives measurable?

The app tells the AI to make objectives measurable without inventing data, so targets come from what you and the client agreed. Add numbers yourself where the session did not set them.

Is there a free AI scribe for treatment plans?

Yes. MentalNote's free plan includes 5 scribe sessions a month, with no time limit and no credit card. The BAA covers the free plan too. Premium is $99 a month or $990 a year.

Will it add a diagnosis I did not give?

The app instructs the AI to give no diagnosis, diagnostic impression or code unless you stated it in the session. Review every draft before you sign it.

Is MentalNote HIPAA compliant?

MentalNote is HIPAA-compliant, with a BAA included on every plan. You accept it in the app before your first session.

Does it update the plan automatically?

No. Record a review session and generate a new Treatment Plan draft, then update the plan in your EHR.
