AI scribe for group therapy

Facts checked on 16 October 2026.

MentalNote is an AI scribe for group therapy: record the group in your browser, get a speaker-labeled transcript and a DAP, BIRP, GIRP, SOAP or Progress draft of the session to review, on a free plan or Premium at $99 a month, with a BAA on every plan. It writes one note per recording, not a separate note for each member.

Group notes are double work: the shared content of the session, then each member's participation. MentalNote drafts the shared part from the recording and puts the transcript beside it, so you can write each member's lines faster.

5 free scribe sessions a month. No credit card. BAA on every plan.

The short answer

  • Record the session in your browser, in the room or on a telehealth call in a browser tab, and get a draft note when the session ends.
  • Pick the format you chart in: DAP, BIRP, GIRP, SOAP, PIRP, Progress, Intake / Biopsychosocial or Treatment Plan, or your own structure with custom instructions.
  • It is told not to fill gaps. The app instructs the AI to write "Not documented in session" for anything the session did not cover, and not to add a denial, a diagnosis or a risk rating you did not state. Check every draft.
  • Start free: 5 scribe sessions a month, no time limit, no credit card. Premium is $99 a month or $990 a year.
  • MentalNote is HIPAA-compliant, with a BAA included on every plan, the free plan too. Many AI scribes offer only a time-limited trial.

Who it is for

Solo clinicians and small practices that want to sign up today, pay a published price and start without a sales call or an IT project. Premium is for solo clinicians. Practices can apply for the Practice Pilot.

How it handles a group session

  • One recording, one draft. The draft covers the group topic, the interventions you used and how the group responded.
  • A speaker-labeled transcript sits beside the draft. Labels are automatic and can be wrong with many voices, so check who said what before you attribute anything to a member.
  • Custom instructions can ask for a short participation line per member, using initials or the IDs you use. The app tells the AI to leave out statements it cannot attribute with confidence.
  • Works for skills groups (DBT, CBT), process groups, psychoeducation and substance use groups, in the room or on a telehealth call in a browser tab.

What a group DAP draft looks like

A human-written, shortened illustration based on a fictional session (no real client). It is not verbatim app output, and your drafts will vary with what is said.

Fictional example: Week 3 of an 8-week DBT skills group (DAP)

Data

Six members attended. Group reviewed distress tolerance homework; four members reported using the TIPP skill at least once. Facilitator taught the STOP skill and led a role play. One member said: "I stopped before I sent the text, and that was new for me."

Assessment

Members are completing homework and starting to use skills outside group. Two members said skills are hard to use at work.

Plan

Next session: radical acceptance. Homework: practice STOP once a day and log it on the diary card.

Risk content is not in this draft because it was not discussed in this fictional group. Each member's own participation note is not in this draft either; you add it.

Per-member notes: what to expect

MentalNote does not split a group recording into a separate note for each member, and it does not copy a note into each member's chart. Most payers and EHRs expect a note in each member's record. Use the group draft for the shared content, then write or edit each member's participation, response and plan yourself, using the transcript.

Get consent from every member before you record, and follow your group's confidentiality agreement.

What it will not do

These are instructions the app gives the AI for every therapy note. They are meant to reduce errors, but they are not a guarantee. Review every line.

What can go wrongWhat MentalNote instructs the AI to do
A risk statement that was never asked ("denies SI")Include risk and safety content only if it was explicitly discussed, and never write that the client denied something the session did not ask about.
A diagnosis the AI guessedGive no diagnosis, diagnostic impression or code unless you stated it in the session.
An intervention you did not useUse only what the session contains: no invented interventions, scores, dates or homework.
A gap filled with typical contentWrite "Not documented in session" for anything the session did not cover.

How it works

  1. Confirm you have your client's consent. The web app asks you to confirm it before every recording.
  2. Record in the browser, in the room or on a telehealth call in another tab.
  3. When the session ends, choose a format and get the draft, with the speaker-labeled transcript beside it.
  4. Review and edit every line, then copy or export it into your EHR.

Prefer Microsoft Word? The optional Word add-in drafts a therapy note from a short typed summary, with no recording.

Frequently asked questions

Can an AI scribe write group therapy notes?

Yes. MentalNote drafts one note per group session from the recording, in DAP, BIRP, GIRP, SOAP, Progress or a custom format, with a speaker-labeled transcript beside it. You review every draft.

Does it write a separate note for each group member?

No. It writes one note per recording. Use it for the shared content, then add each member's participation yourself. Custom instructions can ask for a short line per member by initials; check every attribution.

Do I need consent from every member?

Get consent from each member before you record, as you would for any recording. The web app asks you to confirm consent before every recording.

Does it work for telehealth groups?

Yes. Record a telehealth group that runs in a browser tab, or an in-person group in the room.

Is MentalNote HIPAA compliant?

MentalNote is HIPAA-compliant, with a BAA included on every plan. You accept it in the app before your first session.

Does it connect to my EHR?

Not directly. Copy or export the reviewed note into your EHR or practice management system.
