AI intake note generator

Facts checked on 16 October 2026.

MentalNote is an AI intake note generator for therapists, counselors and psychiatric clinicians: record the first session in your browser, choose Intake / Biopsychosocial, and get a 14-section assessment draft, from presenting problem and history to mental status observations, risk and safety, clinical impressions and plan, on a free plan or Premium at $99 a month, with a BAA on every plan.

An intake is the longest note you write, and it comes after the longest session. MentalNote listens to the first session (with your client's consent) and drafts the biopsychosocial assessment in the order you would write it. Anything you did not cover is marked "Not documented in session", so you can see the gaps to fill at the next visit.

5 free scribe sessions a month. No credit card. BAA on every plan.

The short answer

  • Record the session in your browser, in the room or on a telehealth call in a browser tab, and get a draft note when the session ends.
  • Pick the format you chart in: DAP, BIRP, GIRP, SOAP, PIRP, Progress, Intake / Biopsychosocial or Treatment Plan, or your own structure with custom instructions.
  • It is told not to fill gaps. The app instructs the AI to write "Not documented in session" for anything the session did not cover, and not to add a denial, a diagnosis or a risk rating you did not state. Check every draft.
  • Start free: 5 scribe sessions a month, no time limit, no credit card. Premium is $99 a month or $990 a year.
  • MentalNote is HIPAA-compliant, with a BAA included on every plan, the free plan too. Many AI scribes offer only a time-limited trial.

Who it is for

Solo clinicians and small practices that want to sign up today, pay a published price and start without a sales call or an IT project. Premium is for solo clinicians. Practices can apply for the Practice Pilot.

The 14 intake sections

  • Identifying Information, Reason for Referral and Presenting Problem, History of Presenting Problem.
  • Psychiatric and Treatment History, Medical History and Medications, Substance Use, Family History.
  • Social, Developmental and Cultural History, Trauma History, Strengths and Supports.
  • Mental Status Observations, Risk and Safety, Clinical Impressions, Recommendations and Plan.
  • Privacy by design: identifying information is kept to non-identifying context such as age range, pronouns and living situation. The app tells the AI to refer to "the client" and not to record full names, addresses or contact details.

What an intake draft looks like (shortened)

A human-written, shortened illustration based on a fictional session (no real client). It is not verbatim app output, and your drafts will vary with what is said.

Fictional example: Intake for an adult with low mood after a job loss (selected sections)

Reason for Referral and Presenting Problem

Self-referred. Low mood, poor sleep and loss of interest in hobbies for about 3 months, since being laid off.

Substance Use

Reports 2 to 3 glasses of wine most evenings, up from weekends only. No other substance use reported.

Trauma History

Not documented in session.

Strengths and Supports

Close relationship with a sister, regular runner until recently, previous good response to therapy in college.

Mental Status Observations

Casually dressed, cooperative, speech slow and soft, affect constricted, thought process linear.

Recommendations and Plan

Weekly individual therapy. Client to keep a sleep and activity log. Discuss alcohol use further next session. Treatment plan to be completed at session 2.

Risk and safety content is not shown because this fictional excerpt leaves it out; in a real draft it appears only if you asked about it, and MentalNote is instructed never to write "denies SI" for a question you did not ask. "Not documented in session" shows a topic you can cover next time.

What it will not do

These are instructions the app gives the AI for every therapy note. They are meant to reduce errors, but they are not a guarantee. Review every line.

What can go wrongWhat MentalNote instructs the AI to do
A risk statement that was never asked ("denies SI")Include risk and safety content only if it was explicitly discussed, and never write that the client denied something the session did not ask about.
A diagnosis the AI guessedGive no diagnosis, diagnostic impression or code unless you stated it in the session.
An intervention you did not useUse only what the session contains: no invented interventions, scores, dates or homework.
A gap filled with typical contentWrite "Not documented in session" for anything the session did not cover.

How it works

  1. Confirm you have your client's consent. The web app asks you to confirm it before every recording.
  2. Record in the browser, in the room or on a telehealth call in another tab.
  3. When the session ends, choose a format and get the draft, with the speaker-labeled transcript beside it.
  4. Review and edit every line, then copy or export it into your EHR.

Prefer Microsoft Word? The optional Word add-in drafts a therapy note from a short typed summary, with no recording.
