AI scribe for primary care and family medicine
Facts checked on 15 October 2026.
MentalNote is an AI scribe for solo and small primary-care and family medicine practices: record the visit in your browser, choose the Medical SOAP format, and get a draft with chief complaint, history, review of systems, vitals and exam, assessment and plan to review, on a free plan or Premium at $99 a month.
You run a solo or small primary-care practice. Your visits mix chronic disease follow-ups, acute problems and medication changes, and the notes pile up after clinic. MentalNote turns a recorded visit into a structured draft you review, at a published price, with no sales call.
The short answer
- Record the visit in your browser, in the room or on a telehealth call in a browser tab, and get a draft when the visit ends.
- Choose the Medical SOAP format. Its sections are Chief Complaint, History of Present Illness, Review of Systems, Objective (vitals, exam and results), Assessment and Plan.
- It is told not to fill gaps. The app instructs the AI to write "Not documented." for anything the visit did not cover, and not to add a normal finding, a number, a diagnosis or a code you did not state. Check every draft.
- Start free: 5 scribe sessions a month, no time limit, no credit card. Premium is $99 a month or $990 a year.
- MentalNote is HIPAA-compliant, with a BAA included on every plan.
Who it is for
Solo clinicians and small practices that want to sign up today, pay a published price and start without a sales call or an IT project. Premium is for solo clinicians. Practices can apply for the Practice Pilot.
Built for general medical visits
- The Medical SOAP instructions are written for clinicians in general medical practice: physicians, nurse practitioners, physician assistants, nurses and allied health professionals.
- The Assessment is set up as a numbered problem list, with the status of each problem as you stated it.
- The Plan is set up for medication changes (name, dose and frequency as stated), tests, referrals, follow-up and return precautions.
- The app instructs the AI to keep home readings and vitals exactly as stated, never rounded or corrected.
What the Medical SOAP note looks like
A human-written, shortened illustration based on a fictional test visit (no real patient). It is not verbatim app output, and your drafts will vary with what is said in the visit.
Fictional example: A hypertension follow-up with a cough
Chief Complaint
Hypertension follow-up and a dry cough.
History of Present Illness
3-month hypertension follow-up. Takes lisinopril 10 mg daily; reports missing about 2 doses a week. Home blood pressure readings mostly 140s/90s.
Dry cough for 10 days after a cold. No fever, no shortness of breath, no chest pain. Nonsmoker.
Review of Systems
Constitutional: no fever. Respiratory: dry cough, no shortness of breath. Cardiovascular: no chest pain.
Objective
Vitals: BP 146/92, HR 78, temp 98.4 F, SpO2 98% on room air, BMI 31.
Exam: lungs clear to auscultation bilaterally, no wheeze. Throat mildly erythematous. Heart regular rate and rhythm, no murmur. No ankle edema.
Results: Not documented.
Assessment
1. Hypertension, not at goal, likely partly adherence.
2. Cough, post-viral.
Plan
1. Increase lisinopril to 20 mg daily. Pill organizer. Recheck BMP in 2 weeks for potassium and creatinine. Return in 4 weeks with home BP log. Discussed DASH diet and walking 30 minutes 5 days a week.
2. Honey and fluids. Return sooner if fever or shortness of breath.
The fictional case gives no test results, so Results says "Not documented." instead of a guess.
What it will not do
These are instructions the app gives the AI for every Medical SOAP note. They are meant to reduce errors, but they are not a guarantee. Review every line.
| What can go wrong | What MentalNote instructs the AI to do |
|---|---|
| A normal exam you never did ("lungs clear") | Never record a normal or negative finding that was not stated, and never write "review of systems otherwise negative" unless you said so. |
| A vital sign, dose or result that was never said | Use only what the visit contains, and record vitals, scores and doses exactly as stated. |
| A number the AI worked out | Never calculate, round or convert a number, such as a BMI from height and weight. |
| A diagnosis or code the AI guessed | Give no diagnosis, diagnostic code or billing code (such as ICD-10 or CPT) unless you stated it in the visit. |
| A gap filled with typical content | Write "Not documented." for anything the visit did not cover. |
Also good to know:
- You review every draft. MentalNote writes a draft. You check it, edit it and decide what goes in the record.
- One medical format. Medical SOAP is a general format for medical visits. There are no specialty-specific medical templates.
- Web app only for medical notes. Medical SOAP is in the web app at app.mentalnote.ai. The optional Word add-in is built for therapy notes.
- No direct EHR write-back. Copy or export the reviewed note into your EHR.
How it works
- Confirm you have your patient's consent. The web app asks you to confirm it before every recording.
- Record in the browser, in the room or on a telehealth call in another tab.
- When the visit ends, choose Medical SOAP as the note format and get the draft.
- Review and edit every line, then copy or export it into your EHR.
What it costs
