AI scribe for nurse practitioners
Facts checked on 15 October 2026.
MentalNote is an AI scribe for nurse practitioners in solo and small practices: record the visit in your browser, choose the Medical SOAP format, and get a structured draft to review, on a free plan or Premium at $99 a month, with no sales call.
You may own your practice, or be the only NP in a small clinic, and you write every note yourself. MentalNote turns a recorded visit into a structured draft. You review it, edit it and copy it into your EHR. Psychiatric NPs: see the AI scribe for psychiatry page, which covers the therapy and psychiatric formats.
The short answer
- Record the visit in your browser, in the room or on a telehealth call in a browser tab, and get a draft when the visit ends.
- Choose the Medical SOAP format. Its sections are Chief Complaint, History of Present Illness, Review of Systems, Objective (vitals, exam and results), Assessment and Plan.
- It is told not to fill gaps. The app instructs the AI to write "Not documented." for anything the visit did not cover, and not to add a normal finding, a number, a diagnosis or a code you did not state. Check every draft.
- Start free: 5 scribe sessions a month, no time limit, no credit card. Premium is $99 a month or $990 a year.
- MentalNote is HIPAA-compliant, with a BAA included on every plan.
Who it is for
Solo clinicians and small practices that want to sign up today, pay a published price and start without a sales call or an IT project. Premium is for solo clinicians. Practices can apply for the Practice Pilot.
Built for NP visits
- The Medical SOAP instructions name nurse practitioners among the clinicians they are written for.
- The Objective section has a Results line for point-of-care results you state, such as a urine dip or a pregnancy test.
- The Plan is set up for prescriptions with name, dose, frequency and duration as you said them.
- Return precautions you give the patient go in the Plan.
What the Medical SOAP note looks like
A human-written, shortened illustration based on a fictional test visit (no real patient). It is not verbatim app output, and your drafts will vary with what is said in the visit.
Fictional example: An acute urinary visit
Chief Complaint
Dysuria, urinary frequency and urgency for 2 days.
History of Present Illness
29-year-old female with 2 days of dysuria, frequency and urgency. No fever, no flank pain, no vaginal discharge. LMP 2 weeks ago. No drug allergies.
Review of Systems
Constitutional: no fever. Genitourinary: dysuria, frequency, urgency; no flank pain, no vaginal discharge.
Objective
Vitals: temp 98.9 F, BP 118/74, HR 82.
Exam: mild suprapubic tenderness. No CVA tenderness.
Results: urine dip positive for leukocyte esterase and nitrites. Urine pregnancy test negative.
Assessment
1. Uncomplicated acute cystitis.
Plan
1. Nitrofurantoin 100 mg twice daily for 5 days. Increase fluids. Urine culture sent. Return or call if fever, flank pain or no improvement in 48 hours.
The diagnosis appears because the NP stated it. The draft does not add a code for it.
What it will not do
These are instructions the app gives the AI for every Medical SOAP note. They are meant to reduce errors, but they are not a guarantee. Review every line.
| What can go wrong | What MentalNote instructs the AI to do |
|---|---|
| A normal exam you never did ("lungs clear") | Never record a normal or negative finding that was not stated, and never write "review of systems otherwise negative" unless you said so. |
| A vital sign, dose or result that was never said | Use only what the visit contains, and record vitals, scores and doses exactly as stated. |
| A number the AI worked out | Never calculate, round or convert a number, such as a BMI from height and weight. |
| A diagnosis or code the AI guessed | Give no diagnosis, diagnostic code or billing code (such as ICD-10 or CPT) unless you stated it in the visit. |
| A gap filled with typical content | Write "Not documented." for anything the visit did not cover. |
Also good to know:
- You review every draft. MentalNote writes a draft. You check it, edit it and decide what goes in the record.
- One medical format. Medical SOAP is a general format for medical visits. There are no specialty-specific medical templates.
- Web app only for medical notes. Medical SOAP is in the web app at app.mentalnote.ai. The optional Word add-in is built for therapy notes.
- No direct EHR write-back. Copy or export the reviewed note into your EHR.
How it works
- Confirm you have your patient's consent. The web app asks you to confirm it before every recording.
- Record in the browser, in the room or on a telehealth call in another tab.
- When the visit ends, choose Medical SOAP as the note format and get the draft.
- Review and edit every line, then copy or export it into your EHR.
