AI scribe for occupational therapists

Facts checked on 15 October 2026.

MentalNote is an AI scribe for occupational therapists in solo and small clinics: record the visit in your browser, choose the Medical SOAP format, and get a structured draft that is instructed to keep your grip strengths, range of motion, assist levels and minutes exactly as you stated them, on a free plan or Premium at $99 a month.

You own a small clinic or treat in home health or outpatient, and daily notes eat into treatment time. MentalNote turns a recorded visit into a structured draft for you to review. It uses one general Medical SOAP format, not an OT-specific template, so you stay in control of how functional goals are written.

5 free scribe sessions a month. No credit card. BAA on every plan.

The short answer

  • Record the visit in your browser, in the room or on a telehealth call in a browser tab, and get a draft when the visit ends.
  • Choose the Medical SOAP format. Its sections are Chief Complaint, History of Present Illness, Review of Systems, Objective (vitals, exam and results), Assessment and Plan.
  • It is told not to fill gaps. The app instructs the AI to write "Not documented." for anything the visit did not cover, and not to add a normal finding, a number, a diagnosis or a code you did not state. Check every draft.
  • Start free: 5 scribe sessions a month, no time limit, no credit card. Premium is $99 a month or $990 a year.
  • MentalNote is HIPAA-compliant, with a BAA included on every plan.

Who it is for

Solo clinicians and small practices that want to sign up today, pay a published price and start without a sales call or an IT project. Premium is for solo clinicians. Practices can apply for the Practice Pilot.

What it does for an OT visit

  • The app instructs the AI to keep the measures you state, such as grip strength, range of motion, assist levels and standardized scores, exactly as you said them.
  • Exam findings go under Exam and scored measures under Results, in the Objective section.
  • Functional goals, frequency and home program updates you state go in the Plan.
  • There is no separate treatment section. The interventions you describe are placed within the six Medical SOAP sections, so check where they land.

What the Medical SOAP note looks like

A human-written, shortened illustration based on a fictional session (no real patient). It is not verbatim app output, and your drafts will vary with what is said.

Fictional example: Hand therapy after a distal radius fracture (visit 6)

Chief Complaint

Right wrist stiffness and weakness 8 weeks after distal radius fracture, cast removed 2 weeks ago. Visit 6.

History of Present Illness

Right-hand dominant office worker. Difficulty opening jars and typing more than 20 minutes. Goal: return to full keyboard work and lifting a 10 lb grocery bag.

Review of Systems

Not documented.

Objective

Vitals: pain 3/10 with activity.

Exam: wrist extension 45 degrees, flexion 40 degrees. Mild edema dorsal wrist. Independent with dressing; needs modified independence with jar opening using an adaptive opener.

Results: grip strength right 38 lb, left 72 lb. QuickDASH 41, was 59.

Assessment

1. Stiffness and weakness after right distal radius fracture, improving.

Plan

1. Today: 15 minutes therapeutic exercise (tendon glides, putty grip), 10 minutes therapeutic activity (simulated typing and jar opening), 8 minutes retrograde massage for edema.

2. Continue 2 times a week for 4 weeks. Update home program with putty level progression.

The minutes stay as stated. The draft does not convert them into billing units or add CPT codes.

What it will not do

These are instructions the app gives the AI for every Medical SOAP note. They are meant to reduce errors, but they are not a guarantee. Review every line.

What can go wrongWhat MentalNote instructs the AI to do
A normal exam you never did ("lungs clear")Never record a normal or negative finding that was not stated, and never write "review of systems otherwise negative" unless you said so.
A vital sign, dose or result that was never saidUse only what the visit contains, and record vitals, scores and doses exactly as stated.
A number the AI worked outNever calculate, round or convert a number, such as a percentage, a BMI or billing units.
A diagnosis or code the AI guessedGive no diagnosis, diagnostic code or billing code (such as ICD-10 or CPT) unless you stated it in the visit.
A gap filled with typical contentWrite "Not documented." for anything the visit did not cover.

Also good to know:

  • You review every draft. MentalNote writes a draft. You check it, edit it and decide what goes in the record.
  • One medical format. Medical SOAP is a general format for medical visits. There are no specialty-specific medical templates.
  • Web app only for medical notes. Medical SOAP is in the web app at app.mentalnote.ai. The optional Word add-in is built for therapy notes.
  • No direct EHR write-back. Copy or export the reviewed note into your EHR.

How it works

  1. Confirm you have your patient's consent. The web app asks you to confirm it before every recording.
  2. Record in the browser, in the room or on a telehealth call in another tab.
  3. When the visit ends, choose Medical SOAP as the note format and get the draft.
  4. Review and edit every line, then copy or export it into your EHR.

Want the format on its own? See the medical SOAP note generator.
